PURPOSE &amp; SCOPE: The Manager of Healthy Families Massachusetts (HFM) is a member of the Bridges Homeward Family Support and Stabilization team and helps to manage the operations of Bridges Homeward's Healthy Families program. HFM includes the whole family in service delivery, engaging fathers, mothers, babies, and extended family members. 'Extended family' is defined by the participant and may include significant support systems such as those individuals the family has identified as their kinship network. Through attuned interactions, HFM staff encourage participants at their own pace to develop the knowledge, skills, and support networks that will enable them to be strong, resilient parents with access to the resources needed to successfully manage their lives and be the best advocates for themselves and their children

DUTIES AND RESPONSIBILITIES:
The HFM Manager:

What you will do:
You will be working with the Assistant and Director of Residential Services to provide a broad oversight of your assigned residence. You will function as a role model and an effective leader for the assigned residence, using teamwork as the foundation for the effective functioning of the program. In this role you will provide day to day oversight of the operation and staffing of assigned residence. 
 
As the Program Director, you will maintain the clinical integrity of the program and ensure that the developmental, physical, emotional, and social needs of the consumers are met within the context of a Positive Behavior Support (PBS) model. You will assist the Assistant Director with all program management duties, ensuring that all consumers receive the highest quality of services possible.

The Emergency Residence (ER) is a six-month residential program that supports homeless young adults working towards becoming housed, completing their education, gaining, and maintaining employment, building up their savings, and accessing appropriate physical and behavioral healthcare. The Welcome Center (WC) is a multi-functional program that supports homeless young adults (ages 18- 24) using a cooperative approach to service delivery coordinated through its Day program and Overnight Shelter program.
Bridge is seeking a qualified Manager to supervise the day-to-day operations and full scope of services offered by the ER and WC programs. Expectations of the role include availability to work 2p.m. to 10:00 p.m. or 3p.m. to 11:00 p.m., including (2 weekend shifts per month) weekend coverage
POSITION SUMMARY
The ER/ WC Manager reports to the Associate Director of the Emergency Residence and Welcome Center and is responsible for supervising the day-to-day functioning of the Emergency Residence (ER) and Welcome Center (WC) programs, including supervision of the milieu(s), the delivery of case management services, and maintaining accurate records and case files.
As the Program Manager, you will spend 75% (30 hours per week) of your daily working hours (or as needed) in the milieu, participating in and supervising the staff’s delivery of services to participants utilizing the programs.
The position requires a high degree of initiative and independent judgment. The Program Manager will continually assess and direct a wide range of programmatic issues including staffing, managing, and analyzing the internal workflow, maintaining databases, communicating with staff and residents, transitioning residents in and out of the programs, monitoring residents’ progress, and managing internal and external program relations. The ideal candidate has deep reimbursement knowledge, experience with hands on case management in patient support programs, as well as experience with management of outsourced hub and copay vendors. This is a rare opportunity to join a small, collaborative team supporting two rare oncology products as well as potential future launches.
The Sr. Manager will be responsible for the operations of the Deciphera AccessPoint™ Patient Support Program, which seeks to facilitate a positive patient experience by maximizing patient access and affordability for patients taking Deciphera’s products. The right candidate for this role is solution-oriented, detailed, creative, organized, flexible, and adept at working in an unstructured environment. This will be a critical role, interacting with many internal stakeholders within Deciphera, as well as external vendor partners.
This position will report to the Director Patient Access and Reimbursement as part of the Market Access team and will work either remotely within the US or on-site at our Waltham, MA office. Business travel may be required up to 10% of working hours, including traveling to visit patient support vendors.
What You’ll Do:
<ul>
<li>Ensure ongoing operational excellence of hub and other support program partners.&nbsp; Understand the nuances of individual cases and be able to offer guidance to hub staff as well as recognize trends and patterns that must be addressed to meet the overall needs of patients and the business.</li>
