What are some of the day-to-day duties?
The Acute Coordination Partner is responsible for coordinating and tracking field operations for our Medical Technician Partners (MTPs) as they deliver urgent and routine in-home medical care to seniors. This individual plays a key role in ensuring timely response to urgent needs, proactive outreach to schedule home visits, and optimization of field resources. Additionally, this partner is responsible for scheduling patient visits after they are discharged home from Emergency hospital visits.

The Acute Coordination Partner day-to-day duties will include but are not limited to:
· Serve as central point of contact for incoming requests for MTP visits
· Dispatch Medical Technician Partners (MTPs) to urgent same-day appointments in coordination with Acute Care Team and Center Operations 
· Monitor real-time availability and location of MTPs and to assign the most appropriate responder
· Assess MTP schedules for the upcoming 24-48 hrs and arrange visits to account for geographical efficiency and to provide availability for urgent visit needs 
· Ensure Medical Technician Partners are scheduled at capacity
· Proactively call patients to schedule for home visits according to ideal visit cadence 
· Communicate with patients around expected arrival times, appointment changes and expectations 
· Schedule follow-up appointments for patients discharged home from an Emergency Department visit. 
· Assist in monitoring missed appointments for patients discharged home from the hospital to ensure visits and transportation are scheduled urgently. 
· Collaborate with Clinical Leadership, Acute Care Team, Operations Leaders, and Medical Technician Partners to coordinate daily patient needs and to support process improvement initiatives.

The LTSS Manager reports to the Sr. LTSS Manager and is responsible for the oversight and the day-to-day operations of one or several programs in the Special Programs Department. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

Upper Valley Services (UVS) is one of Vermonts designated non-profit agencies serving people with developmental disabilities and adults who are eligible for Adult Family Care.
UVS is seeking skilled staff to provide direct support to an individual living in the Bradford area. The individual enjoys art, listening to music, playing video games, reading, and doing crafts. Required support includes assistance to maintain health, safety, and well-being, providing necessary therapeutic supports, communicating wants/needs effectively, engaging in the community to explore interests and foster relationships, and increasing independent living skills.
Training and shadowing opportunities will be provided, including de-escalation techniques and Therapeutic Options. The individual requires 2:1 support. Shift hours are 8:00am-4:00pm Monday-Friday.
The ideal candidate must be able to:

<li>Support an individual with complex developmental disability, behavioral, and mental health needs.</li>

The CNA or HHA serves as a member of the PACE Interdisciplinary Team. &nbsp;Under the direction of a licensed nurse, incumbent participates in all aspects of the planning, implementation and evaluation of personal care and safety for the participants at the ADH and in the community. This is a full-time position, Monday- Friday 8-4 position; no nights, weekends or holidays!
 
Responsibilities:
<ul>
<li>Assists participants with all aspects of personal care at the site, and in the community, as directed. &nbsp;</li>
<li>Assists with meals at the site, including 1:1 feedings. &nbsp;Follows all diet and fluid restrictions, and encourages diet compliance. &nbsp;</li>
<li>Assists Nursing with specimen collections, oxygen and vital sign monitoring. &nbsp;</li>
<li>Reports all medical complaints/changes (participant’s physical, cognitive and/or functional status) to the primary nurse.</li>
<li>Serves as an escort for medical, and other pertinent appointments as needed.</li>
<li>Performs all tasks as indicated on daily assignment.</li>
<li>Restocks supplies and monitors the cleanliness of exam rooms, bathrooms, kitchen and supply closets. &nbsp;</li>
<li>Sanitizes tables, chairs, water cooler, exam tables, etc.</li>
<li>Assist with activities, special projects, and therapeutic 1:1. &nbsp;</li>
<li>Performs vital signs and transfers participants safely.</li>
<li>Completes required trainings timely and attends monthly meetings and trainings as required.</li>
<li>Adheres to all policies and procedures.</li>
<li>Maintains and attend CPR training as required.</li>
<li>Ability to pass a fit test. Position requires mask where seal is critical. Incumbent is required to not have facial hair that interferes with a tight seal of the respirator.</li>
<li>Performs other duties, as required.</li>

Qualifications:

<li>High School diploma or equivalent preferred.</li>
<li>HHA certification or equivalent.</li>
<li>Minimum one (1) year of experience with the geriatric population.</li>
<li>Ability to relate well to elders and anticipate their needs.</li>
<li>Ability to perform vital signs, lifting of participants for transfer, and communication of participant changes.</li>
<li>Effective communication, verbal and written.</li>
<li>Consistently works in a positive and cooperative manner with team.</li>
<li>Intermediate Microsoft computer skills preferred.</li>
<li>Frequent local travel Mandarin/Vietnamese/Khmer or Spanish speaking Preferred.&nbsp;</li>
