<li>Provides referrals and supports clients using a variety of culturally, linguistically and educationally appropriate strategies, in a variety of settings. Coordinates with Interpreter Services, as needed</li>
<li>Develops and maintains strong relationships with the community and resources to ensure patient access</li>
<li>Collaborates and coordinates with health care team members, including social work, to promote positive prevention, harm reduction.</li>
</ul>
EDUCATION: Bachelors or Associate's degree (or equivalent combination of formal education and experience in community health) plus a minimum of 1 - 2 years in healthcare is preferred. EXPERIENCE: Preference for 2+ years of experience working in a health care setting, preferably with individuals with substance use disorders.
