Job DescriptionRegistered Nurse – Home Health We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team. As a key member of the care delivery team, you will play a vital role in supporting patients on their health journey—right where they feel most comfortable: at home. Position Summary: The Registered Nurse – Home Health provides personalized, high-quality nursing care to patients in their homes. This role allows for autonomy in your practice, offers flexible scheduling, and provides the opportunity to build meaningful relationships through 1:1 patient care. You’ll work closely with a supportive interdisciplinary team to coordinate and deliver the right care at the right time. Key Responsibilities:
<ul>
<li>Perform initial and ongoing assessments of patients’ physical, psychological, social, and environmental needs.</li>
<li>Develop and implement individualized care plans in coordination with patients, families, and the interdisciplinary team.</li>
<li>Reassess patients regularly and adjust care plans as needed based on changes in condition or circumstances.</li>
<li>Communicate significant findings and changes in patient status to supervisors, physicians, and other care team members.</li>
<li>Maintain accurate clinical documentation in compliance with regulatory and organizational standards.</li>
<li>Uphold patient confidentiality and privacy in accordance with HIPAA and other regulations.</li>
<li>Collaborate with other clinicians to ensure coordinated, efficient, and high-quality care.</li>
</ul>
Qualifications:
<ul>
<li>Current, unrestricted RN license in the state of practice</li>
<li>Minimum of 1 year of nursing experience in a clinical care setting</li>
<li>Valid driver’s license, reliable transportation, and active auto insurance</li>
<li>Willingness and ability to travel within assigned service area (up to 50%)</li>
<li>Associate’s or Bachelor’s Degree in Nursing (preferred)</li>
<li>Prior experience in Home Health or Hospice (preferred)</li>
<li>Familiarity with HomeCare HomeBase or similar EMR systems (preferred)</li>
