 Summary: Under the direction of the Center Nurse Executive/Infection Preventionist/Designee, the Certified Nursing Assistant (CNA) delivers efficient and effective nursing care while achieving positive clinical outcomes and patient/family satisfaction. He/she will function within the standards of practice as accorded by his/her Certification. The CNA performs various patient care activities and related non-professional services essential to caring for personal needs and comfort of patients. The Certified Nursing Assistant may also be responsible for specimen collection for POC testing, where scope of practice, as well as state and federal regulations permit. 
 Responsibilities/Accountabilities: 1. Provides patient care in a manner conducive to safety and comfort. Patient care includes, is not limited to: 1.1. Assists patient with or performs Activities of Daily Living (ADL); 1.2. Obtains and records Vital Signs and weights per policy; 1.3. Assist patients with ambulation and transfers; 1.4. Positions patients in correct body alignment in and out of bed; 1.5. Applies adaptive equipment as ordered; 1.6. Cares for personal belongings; 1.7. Assists physician and/or licensed nurse with treatments and procedures as needed; 2. Makes both occupied and unoccupied beds; 3. Accompanies patients when transported outside the center for appointments as requested; 4. Ensures patients are ready for scheduled rehab therapy and transports patients to rehab area; 5. Notifies licensed nurse if patient identification is missing; 6. Participates in the care planning process and implements care according to care plan; 7. Perform skin treatments, as permitted by state regulation; 8. Applies simple clean dressings, slings, stockings, and support bandages, under direction of the licensed nurse, as permitted by state regulation; 9. Feeds or assists patients with meals and provides additional nourishment and hydrationper care plan; Records patient's oral intake and output; 11. Uses Point Click Care (PCC - POC) according to the Clinical System Processes, if applicable; 12. Answers call light or bell promptly, delivers messages, cleans areas of spillage or accidents; 13. Maintain infection prevention and control precautions practices according to policy. 14. Participates in orientation of new nursing assistants; 15. Reports changes in patient's condition, patient/family concerns or complaints to charge nurse and/or supervisor; 16. Collects and bags soiled linen and delivers to dirty linen area; 17. Obtains clean linens and supplies from supply areas, cleans utility rooms, kitchen areas and all other nursing areas, as assigned; 18. Cleans patient equipment as assigned; 19. Transfers patient's belongings and equipment as directed, and assists with postmortem care; 20. Assists with patient discharge as requested 21. Assists with the orientation of newly hired CNAs; 22. Promotes a culture of safety to ensure healthy practice and living environment; 23. Collaborates and coordinates with other departments to provide timely effective care consistent with individual's needs, choices and preferences; 24. Contributes to an environment that is respectful, team-oriented, and responsive to the concerns of staff, patients and families; 25. Participates in quality improvement activities as requested; 26. Maintains confidentiality and protects sensitive Protected Health Information (PHI) at all times; 27. Stays and works beyond scheduled shift if needed to meet state staffing requirements and/or needs of patients; 28. Performs other duties as requested. 
 Specific Educational/Vocational Requirements: 
 1. Successful completion of a state approved certified nursing assistant program. 2. A minimum of a high school education or high school equivalency diploma is preferred. 3. This position requires that the employee is able to read, write, speak and understand the spoken English language to ensure the safety and wellbeing of our patients and visitors at the work site when responding to their medical and physical needs. 4. Must provide verification of TST (tuberculin skin test) as required by state law and in accordance with Company policy. TSTs will be administered at the work site if required. 5. Basic knowledge of computer use or willingness to learn. 6. Must attend a minimum of 12 hours continuing education programs provided by the center in order to maintain certification. 7. Complies with applicable legal requirements, standards, policies and procedures including but not limited to those within the Compliance and Ethics Program, Standard/Code of Conduct, Federal False Claim Act and HIPAA. 8. Collect nasal specimens for point-of-care testing, following established procedures and manufacturer's recommendations. 2. Maintain specimen integrity by following appropriate infection control practices. 3. Utilize appropriate personal protective equipment (gloves, gown, respirator and face shield/eye protection) for specimen collection. 3.1. Successful fit testing and ability to wear respirator (e.g. N95/KN95) 4. Ensure appropriate labeling of specimens collected. 5. Complete required documentation timely and accurately. 6. Successful completion of Point-of-Care testing specimen collection education and training 7. Successful completion of specimen collection competency 8. Report malfunction of specimen collection equipment or incidents occurring during specimen collection. 9. Successful completion of Point of Care testing specimen processing and analyzing of specimens with POC testing instrument, as permitted by state and federal regulations. 10. Must be able to utilize PPE appropriately, including respirator, gowns, gloves and eye protection. The Infusion Nurse provides primary patient care for specifically assigned patients to include assessments, care planning, evaluation and education of patients receiving infusion therapy and/or other nursing services in accordance with the care matrices and best practices in the home or alternate site setting. Responsibilities include:
<ul>
<li>Provide the direct care for all assigned patients including initiation and maintenance of all communications with appropriate care team members including pharmacists, prescribers, dietitians, home patient representatives/care partner and any appropriate community resource staff members.</li>
<li>Evaluate and education patient/care partner to safely administer the home infusion therapy and provide additional instruction as indicated.</li>
<li>Evaluate the home environment where the care is to be provided and identify interventions that may be required to render the home care environment safe and appropriate for the prescribed care.</li>
<li>Develop in conjunction with other team members the plan of care and evaluate response to therapy with each encounter.</li>
<li>Develop and maintain caseload scheduling via company approved platform per company policy. Communicate any scheduling changes on an as needed basis to appropriate team members.</li>
<li>Coordinate care via approved company platform</li>
<li>Takes part in the nurse on-call rotation to ensure continuous nursing support services to our customers.</li>
<li>Document all communications at time of service as appropriate per Soleo policy.</li>
<li>Strong knowledge of therapies provided by Soleo.</li>
<li>Work in conjunction with appropriate departments to ensure all required patient documents are completed and returned to the branch office within acceptable timeframe.</li>
<li>Ensures timely submission of plan of treatments as required by states guideline.</li>
</ul>
 
Schedule:
<ul>
<li>Per Diem (10 hours per week), Monday-Friday between the hours of 8:30am-5pm, with variable availability for weekends as needed</li>
<li>Must be able to drive up to 1.5 hours to see patients as needed (we offer mileage reimbursement and hourly rate of pay)</li>
<li>Must have knowledge of IV access devices to include: IV Lines, Ports, and administration of Infusion medications</li>
<li>Must be licensed in the state of MA as an RN</li>
</ul>
 
Requirements:
<ul>
<li>RN Degree, BSN Preferred</li>
<li>Advanced certification in specialty areas of practice (e.g., CRNI, ONS, IgRN) preferred but not required.</li>
<li>Administrative Oversight: Responsible for providing administrative oversight, including monitoring of workflows, ensuring compliance with policies, and managing resources efficiently to meet organizational objectives. When working.</li>
<li>Emergency Availability: Must be available to respond promptly to emergencies, ensuring rapid and effective resolution of issues to minimize impact on operations. Availability during non-standard hours, including nights, weekends, and holidays, as required by the nature of the emergency. When working.</li>
<li>Minimum 3 years nursing experience with background in infusion therapy preferred.</li>
<li>Current nursing license for applicable home state</li>
<li>Experience in ICU and/or Cardiac Care preferred.</li>
<li>Proficient with Microsoft Outlook and Microsoft Office Suite.</li>
<li>Valid state issued driver’s license with no significant restrictions on that license.</li>
</ul>
