RESPONSIBILITIES
The Certified Personal Trainer will be responsible for providing one-on-one and couples personalized fitness instruction in person to clients at their home, apartment gym or outside while encouraging them to reach their fitness goals. Responsibilities are as follows:

QUALIFICATIONS

Job Purpose: To provide Long Term Services and Support Coordination to adults enrolled in a One Care Plan. The role is to serve as a liaison to the enrollee in order to access community-based services to improve the quality of life for the enrollee.
Essential Job Function
Duties and Responsibilities: 
<ul>
<li>Provide counseling on the different options available for long term care, which will vary depending on each consumer’s situation. LTSSC includes contacting the consumer to discuss and provide guidance about:
<ul>
<li>the consumer’s medical, physical, emotional, and financial resources,</li>
<li>public, private and informal supports available to meet long term care needs, factors for the consumer to consider in making long term care decisions, including capacity and personal preferences.</li>
</ul>
</li>
<li>Provide timely and accurate assistance with referrals to support the consumer in accessing the agreed upon long term supports.</li>
<li>Follow-up by communicating and consulting with the consumer as needed, to determine the outcomes of previous contacts and to offer additional assistance in locating or using services as necessary.</li>
<li>Complete all documentation (both written and electronically via the WILD data collection system) required for measuring quality and achievement of outcomes.</li>
<li>Work cooperatively with other program staff of the NILP to facilitate the consumer securing needed services and supports.</li>
</ul>
Job Qualifications and Specifications: 
<ul>
<li>Bachelor’s Degree in Social Work, Psychology, Counseling or related field desired</li>
<li>Three to five years' experience in social work, psychology, counseling or related field and experience in coordinating services for one or more of the target populations</li>
<li>Knowledge and understanding of, and sensitivity to, issues concerning the disability community, Independent Living Philosophy and Person-Centered planning philosophy and strategy </li>
<li>Proficient skills with technology including Microsoft 365, Teams, mobile devices, and applications</li>
<li>Strategic thinking skills, active listening, interviewing, and advocacy skills.</li>
<li>Strong oral and written communication skills and excellent listening skills</li>
<li>Work as an effective team member collaboratively through strong individual contribution and commitment</li>
<li>Maintain a positive working relationship with consumers, co-workers, referral sources and others to achieve positive outcomes</li>
<li>Maintain updated computer technology competencies i.e. Microsoft Word, Excel, Outlook</li>
<li>Commitment to continuous improvement strategies and demonstration of the knowledge, skills and abilities related to this position</li>
<li>Participate in continuing education, training, and staff development opportunities</li>
<li>Provide formal and informal transfer of knowledge and peer mentoring leadership</li>
<li>Ability to travel independently</li>
