What You'll do:
As a Home Health RN, you'll deliver skilled, individualized nursing care to patients in the comfort of their own homes- coordinating care, building trust , and making a measurable impact every day.
Key Responsibilities
· Delivers skilled nursing care to patients in their home in accordance with individualized care plans.
· Initiates appropriate preventative and rehabilitative nursing procedures, making necessary referrals to community services.
· Coordinates services, informing the physician, management, and other personnel of changes in the patient's condition and needs.
· Ensures quality and accuracy of patient records in compliance with Medicare and Mass Health standards.
· Conducts ongoing medical documentation, writing OASIS assessments as appropriate.
· Prepares clinical and progress notes, submitting all required paperwork daily. This is a performance requirement.
· Responds promptly to emails and phone calls
What we are looking for:
· Current RN license in the state of Massachusett
· Strong technology skills
· Reliable transportation, valid driver's license, auto insurance
- Strong clinical assessment and documentation skills
