Physical Therapist (PT) – PRN
Wolfeboro Bay Center | Wolfeboro, NH
$55–$60/hour | PRN
We are looking for a Physical Therapist (PT) to provide PRN rehabilitation services at Wolfeboro Bay Center. This opportunity offers flexibility while allowing you to work directly with residents to improve movement, strength, balance, and overall functional ability.
What You'll Do
- Perform physical therapy evaluations and develop individualized plans of care
- Provide skilled interventions addressing strength, balance, gait, transfers, endurance, and mobility
- Establish measurable rehabilitation goals and track resident progress
- Adjust treatment plans based on clinical findings and response to therapy
- Educate residents and caregivers on exercises and safe mobility strategies
- Complete required clinical documentation accurately and on time
- Collaborate with PTAs, OT, SLP, nursing, and other members of the care team

Qualifications
- Active New Hampshire Physical Therapist license required
- Graduate of an accredited Physical Therapy program
- Strong clinical, communication, and organizational skills
- Skilled nursing or post-acute rehabilitation experience preferred, but not required
- New graduate Physical Therapists are welcome to apply
