Healthcare Support is seeking a travel nurse RN Home Health Hospice for a travel nursing job in Bedford, New Hampshire.

Schedule

• Shift: Days
• Hours: 8:00 AM – 5:00 PM (8-hour shifts)
• Days: Monday through Friday
• On-Call: Back-up on-call as needed

Experience Requirements

• Minimum 12 months of hospice experience within the past 3 years (required)
• Experience with Homecare HomeBase (HCHB) required
• Strong background in hospice case management

Territory

• Primary coverage area: Exeter and surrounding communities

Responsibilities

• Manage a hospice caseload and provide ongoing patient assessments
• Coordinate interdisciplinary hospice team (IDG) care plans
• Perform routine visits, symptom management, and end-of-life support
• Conduct documentation and updates in HCHB system
• Educate patients and families on hospice care processes
• Participate in care coordination and communication with providers
