Full-Time, Weekdays Only
A community-based elder care program is seeking a Nurse Practitioner or Physician Assistant to provide primary care to older adults in a supportive, team-based environment.
Responsibilities:

Conduct routine and episodic assessments

Manage acute and chronic conditions

Perform home/facility visits as needed

Provide education and complete timely documentation
-
Participate in limited on-call rotation
Preferred Qualifications:

Licensed NP or PA in Massachusetts
-
Experience with geriatrics
-
Strong communication and clinical skills
