Physical Therapist needed for a skilled nursing facility setting in Hampton, NH. This travel temporary assignment starts on 11/09/2026 and runs for 13 weeks. The selected clinician will earn a weekly pay range of $2,068 to $2,229 with guaranteed hours of 35 per week. The shift is 08:00 to 16:30 (5 days per week). Responsibilities
- Perform comprehensive physical therapy evaluations of residents and develop individualized treatment plans focused on functional outcomes.
- Implement evidence-based PT interventions (therapeutic exercises, gait training, balance, mobility, strength, and ROM activities) tailored to geriatrics.
- Monitor progress, adjust plans as needed, document outcomes, and communicate results to the interprofessional team.
- Provide patient and caregiver education, safety guidance, and discharge planning including home exercise programs.
- Collaborate with nursing, occupancy, and therapy colleagues to coordinate care and ensure quality outcomes.
- Adhere to facility policies, documentation standards, and patient safety protocols.

Requirements
- Active physical therapy license in the state of New Hampshire or eligibility for licensure
- Doctor of Physical Therapy degree or equivalent as required for practice
- Ability to work in a skilled nursing/assisted living setting
- Strong clinical documentation and interpersonal communication skills
- Eligibility for a 13-week travel assignment and ability to work 08:00–16:30 shifts
