About the Role:
Solara Hospice LLC is looking for a compassionate and dedicated Hospice CNA to join our caring team in Waltham, MA. Responsibilities:
- Provide personal care services to hospice patients including bathing, grooming, dressing, and oral hygiene
- Assist patients with mobility, transfers, and positioning to ensure comfort and safety
- Monitor and report changes in patient condition to the supervising nurse or care team
- Deliver compassionate end-of-life care in alignment with each patient's individualized care plan
- Support patients and families with emotional comfort and a reassuring presence
- Document patient care activities accurately and in a timely manner
- Collaborate with interdisciplinary hospice team including nurses, social workers, and chaplains
Requirements:
- Current and valid CNA certification in the state of Massachusetts
- Prior hospice, palliative care, or home health experience preferred
- Strong understanding of end-of-life care principles and patient dignity
- Excellent communication and interpersonal skills with patients and families
- Reliable transportation and ability to travel to patient homes/long term care facilities in the Waltham, MA area
- Ability to work independently while remaining a collaborative team member
- Compassionate, patient, and emotionally resilient demeanor
