Shift Type: Monday-Friday, 8:00 AM - 4:30 PM with weekend rotation
Hours per Week: 40
Bill Rate: $41-$50/hour

Roles & Responsibilities
- Provide comprehensive physical therapy assessments and treatments for residents in a skilled nursing and rehabilitation center.
- Develop and implement individualized care plans that help patients regain mobility, strength, and independence.
- Collaborate with an interdisciplinary team including nurses, physicians, and occupational therapists to ensure holistic care.
- Document patient progress accurately and adjust treatment plans as needed to achieve optimal outcomes.
- Educate patients and their families on therapeutic exercises and strategies for continued recovery at home.
Qualifications
- Active Physical Therapist license in New Hampshire (or eligibility to obtain one).
- Graduate of an accredited physical therapy program (DPT preferred).
- Experience in skilled nursing, rehabilitation, or geriatric settings is a plus.
- Passion for helping others and a collaborative spirit.

Knowledge & Skills
- Knowledge of physical therapy modalities, exercise prescription, and functional training.
- Strong patient assessment and clinical reasoning skills.
- Excellent communication and interpersonal abilities.
- Proficiency with documentation and electronic medical records.
- Ability to work effectively in a team and adapt to changing priorities.
