About Community Health

Community Health is a leading primary care network serving Rutland and southern Addison counties in Vermont. With nearly 400 employees across eight locations, we provide comprehensive healthcare services, including primary care, behavioral health, dental care, pediatric specialties, wellness programs, and walk-in Express Care.

Our integrated, team-based approach brings together medical providers, behavioral health professionals, social workers, and care managers to address the physical, emotional, developmental, and social needs of our patients.

At Community Health, we believe that supporting children and families requires compassionate care, strong partnerships, and a commitment to whole-person wellness.

About the Opportunity

As a Pediatric Licensed Mental Health Clinician, you will have the opportunity to make a meaningful impact on the emotional, behavioral, and developmental well-being of children and adolescents.

You will provide evidence-based mental health treatment, clinical assessments, counseling, and family-centered interventions to help pediatric patients and their caregivers navigate life's challenges.

Depending on the position and organizational needs, opportunities may include on-site, hybrid, or remote work arrangements, allowing eligible clinicians to deliver behavioral health services through secure telehealth platforms while remaining connected to our integrated care teams.

You will collaborate closely with pediatric providers, nursing professionals, behavioral health specialists, and care management teams to ensure children and families receive comprehensive, coordinated support.

Typical Responsibilities May Include:

  • Provide individual, family, and group therapy using evidence-based, developmentally appropriate therapeutic approaches.

  • Conduct comprehensive behavioral health and psychosocial assessments to identify emotional, social, environmental, and developmental needs.

  • Develop and implement individualized treatment plans that reflect each child's unique needs, strengths, and family circumstances.

  • Deliver behavioral health services through in-person appointments or secure telehealth platforms, as appropriate.

  • Offer emotional support and counseling to pediatric patients and families experiencing behavioral health challenges, illness, family stressors, or difficult life transitions.

  • Assess social determinants of health, including family support systems, home environment, financial resources, and access to community services.

  • Collaborate with pediatric providers, nurses, psychologists, social workers, and care managers to coordinate comprehensive patient care.

  • Provide education to children and caregivers regarding coping strategies, emotional regulation, behavioral interventions, and available community resources.

  • Connect patients and families with appropriate behavioral health services, community programs, and additional support resources.

  • Participate in interdisciplinary care planning, case consultations, and treatment coordination.

  • Maintain accurate, timely, and confidential clinical documentation within the electronic medical record.

  • Monitor patient progress and adjust treatment plans based on clinical assessments and family feedback.

  • Stay informed of emerging research, evidence-based practices, and developments in pediatric behavioral healthcare through ongoing professional education.

Specific responsibilities may vary depending on the position, clinical setting, and organizational needs.

Qualifications & Requirements

Required Qualifications:

  • Master's degree from an accredited institution in Social Work (MSW), Marriage and Family Therapy (MFT), Clinical Mental Health Counseling, or a closely related field.

  • Current applicable clinical licensure, such as LICSW, LMHC, LMFT, or LADC.

  • Strong understanding of child development, family systems, and evidence-based pediatric behavioral health interventions.

  • Excellent clinical assessment, communication, and interpersonal skills.

  • Ability to establish trusting, therapeutic relationships with children, adolescents, and their families.

  • Strong clinical judgment, problem-solving, and critical thinking abilities.

  • Ability to collaborate effectively within a multidisciplinary healthcare team.

  • Commitment to ethical practice, patient confidentiality, and compassionate, family-centered care.

  • Ability to adapt to changing priorities within a dynamic healthcare environment.

  • For remote opportunities, ability to provide confidential, professional telehealth services using secure technology and appropriate clinical practices.

Preferred Qualifications:

  • Previous experience providing mental health treatment to children, adolescents, and families.

  • Experience working in pediatric primary care or integrated healthcare settings.

  • Familiarity with trauma-informed care, family-centered interventions, and pediatric psychosocial assessments.

  • Experience coordinating care with schools, community organizations, and other support services.
