Job Overview\n
The Case Management RN will be responsible for managing discharge planning and coordination of care for patients within an acute care environment. This role supports optimal patient outcomes and ensures effective transitions from hospital to home or other care settings.
Schedule\n
This position offers a standard working schedule of 40 hours per week, primarily during the day on weekdays. Scheduling may be adjusted based on operational needs and could consist of either four 10-hour shifts or five 8-hour shifts.
Key Responsibilities\n

- Collaborate with healthcare teams to develop and implement discharge plans for patients.
- Conduct assessments to identify patient needs and resources for discharge planning.
- Facilitate communication among multidisciplinary teams to ensure cohesive patient care.
- Educate patients and families on discharge instructions and available community resources.
- Document case management activities accurately, promptly, and in compliance with regulatory requirements.
- Monitor and evaluate patients’ progress and adjust discharge plans as necessary.

Qualifications\n

- Registered Nurse (RN) with current state licensure.
- At least 2 years of case management experience.
- A minimum of 6 months of experience in discharge planning within an acute care hospital, preferably within a large or teaching hospital environment (500 beds or more).
- Strong interpersonal and communication skills.
- Ability to work collaboratively in a complex healthcare environment.
