Documentation of two measles vaccines, two mumps vaccines, and one rubella vaccine or two MMR vaccines, or proof of immunity via blood test
Strongly preferred record of 3 Hepatitis B vaccines or proof of immunity or declination
Influenza vaccine required during flu season (Sept 15th - April 30th), no exemptions accepted
Documentation of TB skin test screening within 3 months of start date or negative QuantiFERON TB Test or T-spot within 3 months of start date
Physician statement signed by MD/DO/NP/PA within 365 days of start date
MGB Criminal Background Check within 45 days prior to start date
MGB Massachusetts CORI background check within 30 days prior to start date
MGB Resume including past 7 years of work experience with city and state
Respirator Medical Clearance Attestation if fit test is required
2 recent references, minimum 1 supervisor or above in skillset
MGB Skills Checklist required for submission
MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions
MGB Education Verification through background check company
MGB SSN Trace with addresses/aliases for past 7 years
MGB National Sex Offender Check within 30 days of start and updated annually
MGB Exclusion Checks within 30 days of start and annually
MGB TB Symptom Survey if positive PPD, updated annually
MGB Candidate Submission Coversheet required for submission

Background checks and verifications required including criminal, CORI, sex offender, exclusion lists
