Guardian Angel Senior Services is seeking a compassionate, organized, and experienced Registered Nurse (RN) to join our growing Home Care team. What You'll Do
As a Home Care RN, you will play a vital role in ensuring our clients receive exceptional care by:
- Completing initial client assessments and reassessments
- Developing and updating individualized care plans
- Supervising Home Health Aides, CNAs, and caregivers
- Conducting routine supervisory and quality assurance visits
- Providing client and caregiver education
- Coordinating care with physicians, families, and interdisciplinary team members
- Ensuring compliance with state regulations and agency policies
- Accurately documenting visits and clinical notes
What We're Looking For
- Current Registered Nurse (RN) license
- Home care, hospice, or community health experience preferred
- Strong assessment and critical thinking skills
- Excellent communication and organizational abilities
- Ability to work independently while collaborating with a supportive team
- Reliable transportation and valid driver's license
- Proficiency with electronic documentation
