Job DescriptionAbout the Role:Solara Hospice LLC is looking for a compassionate and dedicated Hospice RN to join our team in Waltham, MA. In this meaningful role, you'll provide expert clinical care and emotional support to patients and families during life's most important moments. Responsibilities:
- Conduct comprehensive patient assessments and develop individualized plans of care for hospice patients
- Manage pain and symptom control using evidence-based palliative care practices
- Coordinate with interdisciplinary team members including physicians, social workers, and chaplains
- Provide education and emotional support to patients and their families throughout the end-of-life journey
- Document clinical findings, care plans, and patient progress accurately and in compliance with hospice regulations
- Respond to after-hours on-call needs and urgent patient situations as required
- Collaborate with community healthcare providers to ensure seamless continuity of care
Requirements:
- Active RN license in the state of Massachusetts
- Minimum 1-2 years of nursing experience; hospice, palliative care, or home health experience preferred
- Strong knowledge of pain management, symptom control, and end-of-life care protocols
- Excellent communication and interpersonal skills with patients, families, and care teams
- Ability to work independently in a field-based, patient care setting
- Valid driver's license and reliable transportation required
- CPR certification and current clinical competencies
