Physical Therapist LTC/SNF – Travel Position

Pay: $2,325/week
Job Description
We are seeking a compassionate Physical Therapist to join a skilled nursing and long-term care facility in Lancaster, NH, on a travel contract. The Physical Therapist will evaluate residents, create personalized treatment plans, and provide therapy services aimed at restoring mobility, enhancing strength, and promoting independence. The ideal candidate is patient-centered, adaptable, and experienced in geriatric rehabilitation.
Key Responsibilities
- Conduct evaluations to assess residents’ functional abilities, mobility, and therapy needs.
- Develop and implement individualized treatment plans in collaboration with physicians and interdisciplinary staff.
- Provide therapeutic interventions including gait training, balance activities, strengthening, and mobility support.
- Educate residents, families, and caregivers on therapy goals, safety strategies, and the use of assistive devices.
- Document evaluations, treatment progress, and outcomes accurately in compliance with facility and regulatory standards.
- Collaborate with nursing and therapy staff to deliver coordinated, patient-centered care.
- Support resident independence, safety, and quality of life through evidence-based practices.
Qualifications
- Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy from an accredited program.
- Current and valid Physical Therapist license in New Hampshire (or compact license).
- Minimum of 1 year of recent PT experience, preferably in skilled nursing or long-term care.
- Current certification in Basic Life Support.
- COVID-19 vaccine and recent tuberculosis test.
- Strong assessment, communication, and organizational skills.
- Compassionate approach with experience supporting geriatric populations.
