Position: RN Case Manager
Shift: Days – 7:00 AM–3:30 PM
Schedule: 5 × 8-hour shifts
Guaranteed Hours: 40 hrs/week
EMR: Meditech Expanse – Highly Preferred
Rate: $90/hr
Responsibilities\n
- \n
- Manage healthcare needs and coordinate quality and cost-effective outcomes for patients across the continuum of care.
- Perform admission, concurrent, and discharge assessments.
- Coordinate and monitor services and resources using a multidisciplinary team approach.
- Evaluate appropriate level of care and resource utilization throughout hospitalization.
- Develop and coordinate transition and discharge plans.
- Arrange appropriate post-hospital care and community resources.
- Collaborate with physicians, patients, families, insurance organizations, and government agencies.
- Apply knowledge of Medicare, Medicaid, commercial payers, and reimbursement.

Requirements\n
- \n
- 3+ years clinical experience in acute care – Required
- 3+ years experience in Rehab, Home Health, or Long-Term Care may be considered
- Massachusetts RN License – Required
- BLS – Required
- RN Case Management experience – Required
- Ability to perform comprehensive admission and discharge risk assessments.
- CCM/ACM – Preferred
- Meditech Expanse – Highly Preferred
- Strong communication, collaboration, negotiation, organizational, and analytical skills.
- Must work five 8-hour shifts; 10/12-hour shifts are not available.
- Must reside more than 50 miles from the facility.
