Responsibilities
- Administer prescribed medications and document administration in the resident chart per policy.
- Monitor residents’ conditions, report changes to supervising RN, and coordinate care with the healthcare team.
- Assist residents with activities of daily living (ADLs), including mobility, hygiene, feeding, and comfort measures.
- Support clinical procedures under supervision of licensed staff and ensure patient safety.
- Complete accurate documentation and timely charting; maintain confidentiality and regulatory compliance.
- Observe infection control practices and participate in safety and quality improvement activities.
Requirements
- Active Licensed Practical Nurse (LPN) license in good standing; Vermont licensure or eligibility for licensure in the state of assignment.
- Graduate of an approved practical nursing program.
- Basic Life Support (BLS) certification or as required by policy.
- Minimum 1 year of LPN experience in long-term care or a skilled nursing environment preferred.
- Ability to work Eve and/or Overnight shifts and adapt to a fast-paced setting.
