Responsibilities
- Provide comprehensive physical therapy services to residents in a long-term care setting
- Conduct initial assessments, develop and implement individualized treatment plans, and adjust goals as patients progress
- Deliver therapeutic interventions to improve mobility, strength, balance, gait, and activities of daily living
- Reassess patient progress, modify treatment plans, and document outcomes
- Communicate and collaborate with the interdisciplinary team, including nursing, therapy colleagues, and physicians
- Maintain accurate and timely documentation in patient records and ensure compliance with safety and regulatory standards

Requirements
- Active Physical Therapist license in New Hampshire
- Graduation from an accredited physical therapy program (Doctor of Physical Therapy or equivalent)
- Experience in a skilled nursing facility or geriatric rehabilitation setting preferred
- Ability to work in a long-term care environment and manage a caseload
- Strong communication and documentation skills
- Eligibility for a travel assignment and availability to commit to 17 weeks; 08:00–16:30 schedule flexibility as required
