📍 Barre, VT | 🗓️ 20 Weeks | 🏡 Hospice RN Case Manager

Central Vermont Home Health & Hospice is seeking an experienced Hospice RN Case Manager for a quick-start travel assignment providing compassionate hospice care in a home health setting. This role is ideal for an independent RN with strong case management skills and experience caring for patients and families in rural communities.

The Hospice RN will coordinate and manage patient care plans, perform home visits, collaborate with interdisciplinary teams, provide patient/family education, and ensure high-quality end-of-life care across the continuum.

Schedule is Monday–Friday with holiday coverage required. Prior rural travel experience is preferred. Facility strongly prefers candidates with minimal or no RTO requests during the contract.

Requirements:

Active VT RN License or Compact RN License
Current AHA BLS
Recent Hospice Case Management experience required
Home hospice experience required
Experience with McKesson/Netsmart EMR preferred
Strong independent assessment and communication skills
