Conduct Initial Assessment to determine eligibility for GLSS programs. Develop appropriate service plan based EOEA-mandated assessment tool, and process all paperwork/computer work. Make referrals for Home Care and/or other community services according to the Home Care referral protocol. Coordinate with GLSS Referral Specialist and service providers to ensure timely and non-duplicative service delivery. Contribute to an Intake Team identity and role based on consumer engagement, outreach, motivational interviewing, multi-functional assessment, and consumer driven care.Qualifications:Bachelor's Degree in human services, social work, or related field preferred. Minimum of one (1) year experience in the human services field in lieu of Bachelor's Degree with the ability to obtain a wavier from Executive Office of Aging & Independence (AGE)Bilingual in Spanish, Russian, or Khmer preferredExpert time management with the ability to adjust to changing prioritiesStrong desire to help others and advocate for those needing services and assistanceExcellent communication skills with the ability to establish positive and professional relationships with team members, providers, consumers, and their support systemProficient computer skillsStrong documentation skills with the ability to complete documentation that is timely, accurate, and comprehensiveThe ability to appropriately handle confidential and personal information, meeting the Agency standards for HIPAA compliance and data securityDemonstrates a strong work ethic, dependability, and commitment to consistently meeting responsibilities and deadlinesValid Drivers License and Reliable transportation with the ability to conduct home visits throughout the GLSS service areaHours Full Time 40 hours per week (Monday-Friday) Full Time 40 hours per week (Monday-Friday). Fully In office for the first ninety (90) days of the initial probationary period. Upon successful completion of the probationary period, will require a minimum of one (1) day per week in-office presence with other times spent on community-based assignments and home visits as needed.Pay Range: $22.50-$24.50*GLSS is an affirmative action and equal opportunity employer.
