Responsibilities
- Administer prescribed medications and treatments following physician orders.
- Monitor patient conditions, including vital signs, and escalate changes per protocol.
- Assist with activities of daily living and rehabilitation activities to support mobility and independence.
- Document care in patient records, ensure accurate charting and timely updates.
- Collaborate with the interdisciplinary team (nurses, therapists, dietary and care coordination) to support rehab goals.
- Maintain infection control standards and wound care as assigned, and respond to changes in patient status.

Requirements
- Current Vermont LPN license in good standing
- CPR/BLS certification
- Graduation from an approved practical nursing program
- Ability to work evenings and participate in an eight-week assignment
- Prior experience in long-term care or acute rehab settings preferred
- Ability to comply with facility policies and report changes promptly
