- Performs care assessments, admissions
- Creates an individualized service plan based on the MDS and Physician Summary
- Assisting as necessary in the delivery of other ADHC program services
- Assisting in training ADHC C support staff, including annual in-services
- Reviewing and implementing physician orders
-Completing all required reports promptly
- Completing other duties as assigned.
Education and Work Experience Requirements:
- Computer proficiency (MS Office – Word, Excel, and Outlook, EMR)
- Must be able to work efficiently and meet deadlines while maintaining a positive attitude and providing exemplary customer service
- Ability to work independently and to carry out assignments to completion within the parameters of instructions given
- Associate degree required
- Bachelor's preferred
- Must hold current CPR certificate and current MA LPN license.
