Job Summary:
As a Personal Care Service Provider (PCSP), you will be responsible for assisting individuals with their daily personal needs, ensuring their comfort, safety, and well-being. This role involves assisting the client with non-medical daily living activities. Duties performed may include but are not limited to keeping house (making beds, doing laundry, washing dishes) and preparing meals and providing personal care. The PCSP functions under the direction, instruction and supervision of the RN assigned to the case, Director of Client Services or Director of Operations.
Educational and Professional:
- High School Diploma or GED
- Successful completion of PCSP training program, which meets with State and agency regulations.
- Show evidence of fulfilling the State health requirements.
- Valid drivers’ license
- Must be 18 years or older.
- Successfully complete 12 in-services by December 1 of current year
- 6 Hours initial of Dementia Training and 4 hours yearly continued
Knowledge and Abilities:
- Knowledgeable in-home care rules and regulations
- Be aware of and adhere to all HIPAA rules and regulations and must be an active participant in safety measures for ensuring confidentiality of information as it relates to clients and this agency
- Good oral, written and reading communication skills. Have the ability to understand verbal and written instructions. Read and write documentation.
- Resourcefulness, flexibility, and integrity. Ability to work independently. Ability to function in a team oriented environment
- Ability to make meaningful observations and write brief and accurate reports when necessary
Responsibilities:
- Assist the client with personal hygiene (bathing, shampoo, shaving and oral hygiene)
- Plans and prepares nutritious meals or modified diets as prescribed by a physician.
- Assists with activities of daily living which include dressing, ambulation, toileting, and safe transfer techniques as directed by the Plan of Care.
- Prompts client to take self-administered medications as per the care plan.
- Works with other community agencies involved with the client as directed by the Plan of Care.
- Assists with treatments/therapies as ordered by physician and approved and supervised by RN and Plan of Care.
- Carries out assignments as instructed by PCSP care plan and notifies RN when unable to do so. If RN is unavailable, notify the Director of Client Services.
- Reports changes in clients’ condition or home situation immediately to the office.
- Use of Mobile App for electronic record of visit, including activity, time and mileage
- Performs routine housekeeping duties as related to a safe and comfortable environment for the client as directed by the Plan of Care.
- Accompany the client to necessary appointments, only when authorized by the office.
- Respect the cultural and religious practices of the client and their families.
- Maintain a neat, clean, and orderly appearance *(Dress Code Policy)
- Appropriate footwear for all seasons *(Dress Code Policy). Understands and maintains universal precautions.
- Performs other duties as required by Director Client Services, Administrator, or designee.
- Conduct oneself in a manner that promotes safety
Tools/Equipment:
Hoyer Lift Glucose monitors or meters - Glucometers
Gait Belt Therapeutic ice packs or pillows – Ice caps; Ice collars
Sharps disposal Walker
Wheelchair Cane
Tub seat Transfer/slide board
Physical Demands:
- Lifting, carrying, pushing, or pulling up to 50 pounds
- Patient transfers with no assistance for direct patient care
- Standing, walking, stair climbing, stooping, kneeling, and crouching while administering patient care in the home
- Reaching and handling
- Adequate eyesight for reading, writing, driving, and performing tasks
Aptitudes:
Have the ability to understand verbal and written instructions. Read and write documentation in patient record. Work with the hands and fingers to manipulate large and small objects.
