Sage Group HealthCare Staffing is seeking RN Hospice Case Manager for a travel / contract position in ,Northampton, MA. Accepting both local and traveler candidates.

Pay Rate: Range $2120 to $2200 Weekly Gross pay based on a 40-hour work week (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.54/mile for distance covered between patient home appointments
Unit: Home Health Hospice
Shifts available: Days
Shift timings: 8 Hours; Days, 8a – 5p
Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri, No weekends.
Position Requirements:
- Registered Nurse: 2 years - Required
- Recent/Current Hospice nursing experience in a home health setting: 1 year within the past 3 years - Required
- Home Care Home Base (HCHB) charting experience - Required
- Must have valid Driver’s License and provide proof of personal Auto Insurance
- Must have reliable vehicle for patient visits
- Must be Covid Vaccinated or provide medical or religious exemption
Education, License and Certification Requirements:
- ASN or an equivalent – Required
- BSN or an equivalent – Highly preferred
- RN License for State applying or Compact if accepted - Required
- BLS Certification from AHA or American Red Cross - Required
Job Description:
- Provide and coordinate care for assigned hospice patients in comfort of their homes.
- Develop and implement nursing care plans including Start of Care and admissions as assigned.
- Case management and provide clinical oversight to interdisciplinary
- Provide nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
- Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
- Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
- Notify physicians of significant changes in status
- Document and complete charting for all patient visits within 24 hours.
- Provide Bereavement Support to patient families.
