Physical Therapist needed for a travel assignment in a skilled nursing facility setting in Rochester, New Hampshire. Start date: 11/23/2026. The assignment runs for 13 weeks with guaranteed hours and a weekly pay range of $2,389 to $2,569. Shifts are 5 days per week, approximately 8:00 a.m. to 4:30 p.m., totaling 35 guaranteed hours per week. Work setting is a long-term care facility, focusing on rehabilitation within geriatric and post-acute populations.

Responsibilities
- Assess residents’ physical therapy needs and develop individualized treatment plans in a SNF setting
- Implement and monitor therapeutic interventions to improve mobility, gait, strength, balance, and activities of daily living
- Reassess and modify plans of care based on resident progress and multidisciplinary input
- Document evaluations, treatments, progress notes, and discharge planning in accordance with facility policy
- Collaborate with nursing staff, physicians, and other rehabilitation professionals to optimize patient outcomes
- Educate residents and families on therapy goals, home exercise programs, and safety strategies

Requirements
- Active physical therapy license to practice in New Hampshire
- Doctor of Physical Therapy (DPT) degree or equivalent professional degree with eligibility for licensure
- Ability to work in a skilled nursing facility environment and participate in a multi-disciplinary care team
- Strong communication skills and documentation accuracy
- Prior experience in geriatric or SNF rehabilitation is preferred
- Willingness to support a 13-week travel assignment starting in late November
