The Nurse Trainer ensures clients receive optimal medical care and oversees safe medication practices in staffed or shared community homes. This role provides medical oversight, medication consultation, and training to providers, staff, and families, while ensuring compliance with He-M 1201 regulations and the 404 Nursing Delegation Rule.
Essential Responsibilities
Medication Oversight & Consultation
- Provide medication consultation to providers, staff, and families for individuals on caseload
- Maintain up-to-date medical records, including:
- Current medication lists
- Copies of physical examinations
- Provide education on medications, side effects, and related concerns
- Attend team meetings as required
- Inform Residential Managers of medication-related issues
- Complete documentation in accordance with agency policies and procedures
High-Risk & Medical Protocol Development
- Develop protocols for individuals with high medical or safety risks
- Collaborate with the Nursing Team and Case Manager to create individualized care plans outlining:
- Staff responsibilities
- Required training (e.g., G-tube care, seizure management, cast care, diagnostic-specific supports)
Regulatory Compliance (He-M 1201)
- Ensure adherence to He-M 1201 medication regulations
- Conduct Medication Quality Assurance (QA) reviews:
- Family homes: every 1-3 months
- Staffed residences and day programs: monthly
- Distribute QA review copies to programs and maintain nursing files
- Ensure annual medication re-certification for all staff/providers:
- Observe medication administration for competency
- Assess individuals’ ability to self-administer medications per He-M 1201
- Assist in developing medication training plans as needed
Health Monitoring & Care Coordination
- Monitor client medical, dental, and psychiatric care
- Promote best practices for effective health supports
- Serve as liaison to physicians, mental health centers, and hospitals
- Notify Case Manager or Dental Coordinator when dental care is needed
- Coordinate diabetic training for staff with qualified instructors
Medication Occurrence & Error Management
- Document, monitor, and assess medication occurrences
- Review medication occurrences with staff/providers
- Coordinate follow-up with supervisors or coordinators as needed
- Complete:
- Monthly Medication Error Reports
- Medication Action Reports every 6 months (per He-M 1201)
Meetings & Department Participation
- Attend:
- Nursing Department meetings including state nurse trainer monthly meetings
- Individual Service Agreement (ISA) meetings, when required
- Other designated meetings
General Responsibilities
- Teamwork, regular office attendance, and face-to-face interaction are essential job functions
- This position requires an initial in-office presence of 3–4 days per week to support onboarding, collaboration, and training.

Qualifications
Education & Experience
- Unencumbered New Hampshire Registered Nurse (RN) license (required)
- Minimum two years of nursing experience preferred in the ID/DD field; acute care or hospital experience may be accepted in lieu of ID/DD experience
- He-M 1201 Nurse Trainer Certification (required within 3 months of hire)
- Valid NH driver’s license and reliable vehicle during work hours
- Proficiency in Microsoft Office Suite and database systems

All staff must adhere to the NH state law requiring mandated reporting of suspected abuse, neglect, or exploitation. Additionally, staff are required to maintain criminal, motor vehicle and Bureau of Elderly and Adult Services record checks that meet agency standards. Staff are also required to travel and transport clients when applicable. This includes having access to a reliable vehicle, a valid driver’s license and maintaining personal auto insurance of $100,000/300,000.
