- Monday-Friday, 8-4:30p for 40 hrs/wk, 13 weeks
- 4-5 location visits/day
- Could go into homes, LTC, assisted living facilities for patients
- Must have at least 1 year hospice case manager experience
- Coverage area: Natick/Framingham area
- First 2 weeks will be paid $65 Blended/hr
