Job Description

Job DescriptionAfter-Hours Hospice Registered Nurse (RN)About the Role

We are seeking a compassionate, reliable, and experienced Registered Nurse to provide after-hours hospice care and support. This position is ideal for a nurse who understands that hospice care extends beyond normal business hours and is committed to being available when patients and families need guidance, comfort, and clinical expertise most.

Hospice is a 24-hour care model, and our responsibility to patients continues regardless of the time of day or where they call home. Families, caregivers, facility staff, and patients rely on us for support during some of their most challenging moments. We believe every call represents a need for assistance, education, reassurance, or clinical intervention, and we strive to respond with urgency, compassion, and professionalism.

The After-Hours RN plays a critical role in ensuring continuity of care by providing telephone triage, symptom management, patient visits, clinical assessments, and support during evenings and weekends. This nurse works closely with physicians and the interdisciplinary team to address changes in condition, urgent concerns, and end-of-life needs as they arise.

We are looking for someone who shares our patient-centered approach to hospice care. The ideal candidate is clinically confident, compassionate, and responsive, with a commitment to supporting patients and families wherever they are in their journey.
