Job Title: Case Manager

Job Description

As a Case Manager, you will be responsible for building strong communication and relationships with clients and their associated partners, which include schools, funding partners, therapists, agencies, clinicians, supervisors, and foster homes. You will set goals and limits for youth, develop interventions, and ensure compliance with policies and procedures. Your role will involve initiating and monitoring treatment plans, participating in on-call services, representing the agency on community boards, and attending meetings. You must maintain the highest level of confidentiality and document all work accurately.

Responsibilities

  • Communicate and build relationships with clients and collateral partners.
  • Set limits for youth and develop interventions.
  • Initiate and monitor treatment planning for clients.
  • Ensure compliance with policies and procedures.
  • Interact effectively in reporting as directed for computer and paper reports.
  • Participate in on-call services as needed.
  • Represent the agency on task forces and community boards.
  • Attend meetings within the agency and with collateral partners.
  • Stay informed on trends and significant events in Human Services and Case Management.
  • Analyze situations, identify problems, and evaluate alternative courses of action.
  • Plan and organize work effectively to ensure completion.
  • Use discretion and judgment under supervisor's guidance.
  • Maintain confidentiality at all times.
  • Document all work responsibly, accurately, and completely.
  • Perform other duties as assigned related to these responsibilities.

Work Environment

This role involves working in a typical office setting as well as individual homes with environments unknown to the agency. The noise level is generally low to moderate. The schedule is Monday to Friday, from 9 am to 5 pm, with potential for additional hours on weekends if needed. The organization is committed to environmental sustainability, integrating green initiatives into all operations. Benefits include tuition reimbursement, 2 weeks of PTO to start, 12 paid holidays, up to 5 weeks of PTO after 10 years, and 55 cent mileage reimbursement.

Job Type & Location

This is a Permanent position based out of Malden, MA.

Pay and Benefits

The pay range for this position is $51000.00 - $52000.00/yr.
