The Roadmap to Reunification Specialist (R2R) serves as a liaison for the clinical team, NH DCYF, birth parents, caregivers, and all collateral services and providers and manages/coordinates all related treatment services and program documentation.

Responsibilities:

  • Serve as a liaison for the clinical team, the client, the family, and all collateral service providers
  • Conducts initial ice breaker meeting within 10 calendar days of case opening
  • Document per New Hampshire state regulations, including daily progress notes
  • Meet with the treatment team at minimum every 30 days and caregivers to ensure proper implementation of the treatment plan, address specific target areas, review overall progress toward goals, and discuss changes to the treatment plan if needed
  • Ensure compliance, accuracy and quality of documentation and clinical records
  • Ensure compliance with Federal, State and agency guidelines related to administrative and clinical policies
  • Ensure clients receive quality and professional services
  • Respond to parent/caregiver/family concerns and determine appropriate resolutions
  • Participate in Clinical Team Meeting and other meetings as assigned

This is a Full Time, 40-hour per week position.

Qualifications:

  • High school diploma required
  • At least 2 years of experience working with children and adolescents identifying barriers and resources to reunification with families
  • Demonstrated knowledge of the needs of children and families in the child welfare system
  • Bachelor's degree preferred, with a major in social work, counseling, psychology, or a related human services field
  • Valid driver’s license with reliable and consistent access to a vehicle
