We’re seeking a Registered Nurse Care Manager to join a dynamic care management team dedicated to supporting people with complex and chronic conditions. This isn’t just bedside care. It’s holistic, hands-on care management that blends clinical skill, patient advocacy, and real community impact. You’ll meet patients where they are — whether in a clinic, in the community, at home, or by phone — and guide them step-by-step through their health journey.


What You’ll Do:

  • Conduct comprehensive assessments and develop tailored care plans
  • Use motivational interviewing to engage and empower patients
  • Coordinate care across multiple settings and providers
  • Provide education, medication reconciliation, and discharge planning support
  • Connect patients to primary care, behavioral health, social services, and community resources
  • Collaborate closely with an interdisciplinary team to remove barriers to care


The Schedule & Setting:

  • Hybrid model — flexibility to work remotely and in the community
  • Regular team meetings at local practice locations
  • Opportunities to directly impact patients across diverse care settings


Qualifications:

  • Active MA RN license (LPN with strong Care Management background considered)
  • 3–5 years of nursing experience (home health, ambulatory care, community/public health, or case management ideal)
  • Bilingual in Spanish strongly preferred
  • Care Management certification a plus (CCM, ANCC RN-BC)
  • Valid driver’s license
