**Join Our Team as a Home Care Physical Therapist!** **Job Description:** - Are you passionate about helping individuals regain their mobility and independence? - As a Physical Therapist, you'll provide one-on-one therapy care for patients in their own homes! **Primary Job Responsibilities:** - Assess each patient’s condition and develop personalized care plans to reach their goals and maximize independence. - Collaborate with an interdisciplinary team to ensure each patient’s needs are met effectively. - Demonstrate proficiency in standard therapy measurement techniques, including ROM, strength, and balance tests. - Utilize a diverse array of therapeutic modalities to enhance the care provided to clients. **Requirements:** - Vermont PT licensure - Home Health experience and OASIS experience required for case management - Experience with McKesson/Netsmart EMR strongly preferred - Seeking candidates with no requested time off **Shift Information:** - Tuesday to Saturday, 8 AM - 5 PM **Minimum Years of Experience:** - 1 year **Certifications / Requirements:** - ACLS - BLS/BCLS - PALS - NRP - ASHA - CHT - ACP Training **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
