Job Summary
- We are seeking a Patient Care Coordinator to support patients and healthcare providers by coordinating appointments, maintaining accurate records, and providing excellent customer service.
- The ideal candidate will have at least 1 year of experience working with Epic EMR in a medical setting and a strong healthcare/customer service background.
Key Responsibilities
- Schedule patient appointments, manage provider calendars, and coordinate follow-up visits.
- Serve as a primary point of contact for patients, families, and healthcare providers.
- Greet patients and visitors and provide a welcoming, professional experience.
- Answer patient questions and provide basic information regarding care plans and services.
- Update and maintain accurate patient information and records in Epic EMR.
- Coordinate communication between patients, providers, and other healthcare team members.
- Assist with referrals, follow-ups, and other patient care coordination activities.
- Protect patient confidentiality and follow applicable healthcare privacy requirements.
- Perform other administrative and patient support duties as assigned.
Qualifications
- 1+ year of experience using Epic EMR.
- 1+ year of experience working in a medical/healthcare setting.
- Strong customer service and communication skills.
- Excellent organization and attention to detail.
- Ability to manage multiple appointments and priorities in a fast-paced environment.
