This is a travel, temporary assignment supported by Healthforce Healthcare LLC. The role requires delivering hands-on nursing care in a long-term care environment, coordinating with the care team to meet resident needs during evening and overnight shifts.

Responsibilities
- Provide direct, patient-centered nursing care to residents during evening and overnight shifts, including monitoring status and responding to changes in condition.
- Administer prescribed medications and treatments in accordance with physician orders and facility policies.
- Perform routine assessments, document findings, and escalate any clinical concerns to the supervising RN or physician promptly.
- Assist with activities of daily living, wound care, and rehabilitation support as part of the interdisciplinary team.
- Maintain accurate and timely documentation in the electronic health record; ensure accuracy of MARs, flowsheets, and care plans.
- Ensure resident safety, infection prevention practices, falls prevention, and adherence to resident rights and privacy standards.

Requirements
- Active Licensed Practical Nurse (LPN) license in the state of Vermont
- Ability to work evening and/or overnight shifts in a long-term care setting
- Graduation from an approved practical nursing program
- Experience in a long-term care or skilled nursing environment preferred
- Ability to pass orientation and comply with facility policies and travel assignment requirements
