Responsibilities
- Conduct speech, language, cognitive, and swallowing assessments for residents in an acute care–oriented SNF setting
- Develop, implement, and monitor individualized therapy plans aligned with patient goals and medical status
- Provide direct therapy services, document progress, and adjust treatment plans as needed
- Collaborate with an interdisciplinary care team (nursing, rehabilitation, social work) to support discharge planning and overall patient outcomes
- Educate patients, families, and care staff on communication strategies, feeding safety, and therapy goals
- Ensure timely and accurate documentation in the medical record and maintain compliance with regulatory standards

Requirements
- Active, unrestricted license to practice Speech Language Pathology in New Hampshire or eligibility for licensure
- Master’s degree in Speech-Language Pathology from an accredited program
- Experience or comfort working in acute care or skilled nursing facility settings is preferred
- Ability to perform duties in a clinical setting with appropriate documentation and collaboration with the care team
- Strong interpersonal skills to communicate with patients, families, and colleagues
- Ability to meet the physical and ergonomic demands of a SNF therapy role and to work within a daytime, weekday schedule
