Job Requirements\n
- \n
- Active Massachusetts RN license
- Minimum of 1 year of nursing experience required, or 6 months of nursing experience with a BSN
- Current CPR certification required
- Successful completion of the pre-hire medication test
- Home health, home care, and IV experience preferred
- Strong computer skills preferred
- BSN preferred
- Ability to provide high-quality care independently while maintaining excellent documentation standards

Responsibilities\n
- \n
- Complete comprehensive nursing assessments upon admission, including physiological, psychosocial, environmental, and health history factors
- Develop, implement, and revise individualized plans of care based on patient needs
- Maintain accurate and timely documentation, including narrative notes, admission paperwork, and discharge summaries
- Coordinate care with physicians, interdisciplinary team members, and community resources as appropriate
- Monitor patient progress and communicate significant changes in condition to the appropriate care providers
- Provide and document supervision of the Home Health Aide plan of care every 14 days
- Obtain and clarify valid provider orders for all care delivered
- Support discharge planning and ensure all required discharge paperwork is completed appropriately
- Participate in interdisciplinary case conferences and contribute to effective care planning
- Perform all other duties as assigned in support of safe, skilled home care
