Role Description The Licensed Practical Nurse (LPN) is a full-time, on-site role based in Chelmsford, MA or Dracut, MA. The LPN provides direct nursing care and health oversight to individuals receiving services, including administering medications, monitoring health status, and implementing care plans under appropriate clinical supervision. Day-to-day responsibilities include documenting health information, responding to urgent medical needs, coordinating with physicians and other healthcare providers, and educating individuals and support staff on health-related topics. The LPN collaborates with interdisciplinary team members to ensure safe, high-quality care, adheres to regulatory and agency policies, and supports individuals in achieving greater wellness and independence.

Qualifications

- Current and unrestricted Licensed Practical Nurse (LPN) license in the state of Massachusetts.
- Experience providing direct nursing care in community-based, residential, or human services settings is preferred.
- Strong skills in medication administration, health monitoring, and implementation of nursing care plans.
- Ability to accurately document clinical information and maintain confidential records in compliance with regulations.
- Effective communication and collaboration skills for working with individuals served, families, and interdisciplinary teams.
- Demonstrated ability to exercise sound clinical judgment, prioritize tasks, and respond to medical emergencies.
- Basic computer proficiency and comfort using electronic health records or similar systems.
- Commitment to person-centered support, cultural sensitivity, and non-discriminatory care practices.
- Ability to work on-site in Chelmsford, MA, including potential evening, weekend, or holiday shifts as needed.
