Job DescriptionHospice Registered Nurse (RN)
Acclaim Home Health is seeking a compassionate and skilled Hospice Registered Nurse (RN) to provide high-quality, patient-centered end-of-life care to hospice patients in Worcester, MA. The Hospice RN plays a critical role in managing patient care, supporting families, and collaborating with the interdisciplinary hospice team to ensure comfort, dignity, and quality of life.
This position is ideal for an RN who is passionate about hospice care and making a meaningful impact for patients and their loved ones.
Responsibilities
- Provide skilled nursing care to hospice patients in accordance with individualized care plans
- Perform comprehensive nursing assessments and ongoing evaluations of patient status
- Manage pain and symptom control to promote patient comfort and quality of life
- Educate patients and families on disease progression, medications, and hospice services
- Coordinate care with physicians and the interdisciplinary hospice team
- Administer medications and treatments in accordance with physician orders and hospice protocols
- Provide emotional support and guidance to patients and caregivers during end-of-life care
- Participate in interdisciplinary team meetings and care conferences
- Maintain accurate, timely clinical documentation in compliance with hospice regulations
- Ensure compliance with state, federal, and hospice regulatory requirements
Qualifications
- Active Registered Nurse (RN) license in the state of Massachusetts
- Graduate of an accredited nursing program
- Hospice, home health, or palliative care experience preferred
- Strong clinical assessment, critical thinking, and communication skills
- Ability to work independently and collaboratively within an interdisciplinary team
- Compassionate, patient-focused approach to end-of-life care
- Valid driver’s license, reliable transportation, and ability to travel locally
