Job Description
POSITION SUMMARY:
\n To assist in implementing a medically-directed plan of care for patients in their homes and to assist patients and their families with a variety of home management tasks and personal care activities
\n MINIMUM QUALIFICATION STANDARDS:
\n I. Education:
\n Basic Education (high school)
\n II. Experience:
\n Combination of education and experience from which comparable knowledge and skills are acquired
\n III. License Requirements:
\n N/A
\n IV. General:
\n • Must demonstrate and maintain any required competency
\n • Able to speak, read and write in English
\n • Be honest, self-directed, possess initiative, display good judgment and a positive attitude toward daily activities
\n • Have the ability to communicate clearly and effectively both orally and in written work
\n • Be able to work cooperatively and in a productive manner with organization’s employees, clients, customers and the general public
\n • Maintain satisfactory standing with the Adult Abuse Registry, Child Abuse & Neglect Registry, Vermont Criminal Information Center and OIG
\n • Be free from the illegal use and effects of drugs and alcohol in the workplace
\n • Be able to carry out the functions of this job without risk of harm to the health and safety of self and others
\n • Hold a valid driver’s license and have a reliable, registered vehicle with insurance as required by the organization, and appropriate to position
\n • Supervision is received from RNs and clinical manager/director or designee
\n WORKING CONDITIONS:
\n 1. Traveling may be required
\n 2. May work in community based settings, and clients’ homes and outdoors
\n 3. May work during non-business hours
\n 4. May experience:
\n • Hostile and emotionally upset clients, family members, visitors and staff
\n • Stress due to a demanding profession
\n • An exposure to blood-borne pathogens
\n • A variety of environmental exposures such as inclement weather, animals, firearms, tobacco use, dust, and other client based activities and preferences
\n ESSENTIAL PHYSICAL REQUIREMENTS:
\n 1. Must walk, sit, stand, bend, lift and move intermittently during working hours
\n 2. Must have the ability to push, pull and lift from the floor, up to a total of 50 pounds and be able to move with the weight, as to organization’s standards
\n 3. Climbing-be able to navigate stairs of varying heights and degrees
\n 4. Balancing-be able to maintain balance
\n 5. Kneeling, crouching, stooping, twisting, reaching below waist level, reaching overhead and reaching torso level
\n 6. Manual finger dexterity-will need dexterity to perform duties and use equipment
\n 7. Visual acuity-will need sufficient eyesight to drive a car, use office equipment and perform duties
\n 8. Auditory acuity- will need to assist individuals in person and by telephone
\n PERFORMANCE REQUIREMENTS:
\n I. Performance Standards:
\n • Meets the attached Hospice Values Competencies at a proficient level
\n • Has the skill to provide service across locations such as UVMHN McClure Miller Respite House Setting and Home setting
\n • Acts as a peer resource
\n • Implements various aspects of a medically-directed plan of care that do not require a nursing license, such as: non-sterile wound care, range of motion, etc.
\n • Assists with personal hygiene, elimination, feeding and mobility according to the patient’s needs and the nursing orders and plan of care
\n • Provides education to family about personal care and safety per the plan of care and comprehensive assessment
\n • Communicates observations and changes in patient status with the RN case manager or designee
\n • Documents the care provided and the patient’s response to care
\n • On a limited bases, performs general housekeeping
\n a. Light everyday housecleaning tasks: make beds, wash dishes and clean up kitchen and bathroom areas and make tidy living areas
\n b. Light laundry
\n • Provides input on presented clients and families at interdisciplinary team meetings
\n • Attends/Completes assigned trainings
\n • Attends staff meetings and in-service programs
\n • Other duties as assigned by supervisor

\n • Performs duties as assigned that may be outside his/her principle responsibilities
\n • Meets own learning needs. Participates in educational programs of the organization, including completion of all mandatory competencies, education or training
\n • Effectively communicates to transfer information, provide guidance and resolve conflicts
\n • Reports concerns and complaints regarding abuse, neglect and exploitation in accordance with Vermont State Law and the organization
\n • Enhance or improve organization’s services by participating in team meetings, projects and committees

\n • Documents the provision care and services in accordance to organization’s policy
