DESCRIPTION:
Job Title: Experienced Home Health RN – Central Vermont
Location: Barre, VT 05641
Schedule: 3 days per week, 12-hour shifts
Position Type: Contract
Responsibilities:
- Provide skilled nursing care in patients’ homes
- Perform assessments and develop individualized care plans
- Complete OASIS documentation accurately and timely
- Manage patient caseloads efficiently, coordinating with the interdisciplinary team
- Educate patients and families on disease management, medications, and health maintenance
Qualifications:
- Valid RN license in Vermont
- Proven home health and case management experience required
- OASIS experience required
- Strong preference for nurses with travel experience
- Must be available for the full duration of the contract with no requested time off
