Job Title: Registered Nurse Care Manager

Location: Multiple Locations Across Massachusetts

Duration: 3 Month (Possible to hire)

Start Date: Immediately

Job Description:

  • We are seeking a licensed Registered Nurse (RN) to serve as a Care Manager in a community-based role supporting dual-eligible Medicare and Medicaid members with complex medical, behavioral, and social needs. The Care Manager will conduct in-home and community-based visits to perform comprehensive assessments, including UCA (Uniform Core Assessment) and functional evaluations, to support individualized care planning and improve health outcomes.


Key Responsibilities:

  • Conduct in-home and community-based member assessments to identify care needs and support care coordination
  • Perform comprehensive evaluations, including UCA (formerly MDS) and functional assessments
  • Develop and implement individualized care plans based on assessment findings
  • Coordinate services across medical, behavioral health, and social support systems
  • Collaborate with multidisciplinary teams to ensure continuity of care and address complex care needs
  • Maintain accurate and timely documentation in compliance with regulatory and organizational standards
  • Required Qualifications:


Active Massachusetts RN License

  • Associate’s Degree in Nursing
  • Minimum of 3 years of experience as an RN or Behavioral Health Clinician, OR
  • 1 year of RN/Behavioral Health experience combined with 2 years of experience working with complex populations
  • Valid Massachusetts driver’s license, reliable transportation, and current auto insurance


Preferred Qualifications:

  • Experience with Long-Term Services and Supports (LTSS) and dual-eligible populations
  • Background in Medicare/Medicaid managed care environments
  • Experience in population health, community health, or behavioral health settings
  • Proven success in complex care coordination and case management
  • Case Management Certification (CCM) preferred


Work Environment:

  • Community-based and home visit setting
  • Travel required across multiple locations in Massachusetts
  • Flexible scheduling to accommodate member needs
