Job Description
Join Our In-House Therapy Team at The Center for Living and Rehab!
 We are currently seeking a PRN Physical Therapist (PT) to join our in-house rehab team. At The Center for Living and Rehab, we take pride in delivering high-quality, resident-centered care in a supportive and team-oriented environment.
Responsibilities:
<ul>
<li>
Evaluate residents and develop individualized physical therapy treatment plans
</li>
<li>
Deliver skilled therapy services to help improve mobility, strength, and overall function
</li>
<li>
Monitor progress and adjust treatment plans based on resident response
</li>
<li>
Educate residents, caregivers, and staff on therapy goals and safety techniques
</li>
<li>
Document evaluations, treatment sessions, and outcomes accurately and timely using electronic medical records
</li>
<li>
Collaborate with interdisciplinary team members to support resident care plans
</li>
</ul>
Qualifications:
<ul>
<li>
Graduate of an accredited Physical Therapy program
</li>
<li>
Active Vermont Physical Therapist license (required)
</li>
<li>
Previous experience in SNF or long-term care preferred
</li>
<li>
Strong communication, organizational, and clinical skills
</li>
<li>
Experience with EMR systems preferred&nbsp;
</li>
