Responsibilities
- Deliver direct nursing care to residents following individualised care plans and facility policies.
- Administer medications and treatments safely within LPN scope of practice.
- Monitor, document, and promptly report changes in resident condition to the supervising RN and provider.
- Collaborate with CNAs and the interdisciplinary team to support resident goals and care plan compliance.
Requirements
- Active, unencumbered LPN licence in the applicable state.
- Minimum 1 year SNF, LTC, or post-acute experience preferred.
- Current BLS/CPR certification.
- Reliable, detail-oriented, and committed to compassionate resident care.
