Ardor Health Solutions is seeking an experienced Physical Therapist for a Home Health travel assignment with Amedisys in Londonderry, New Hampshire.
\n
This is a great opportunity for a PT who enjoys autonomy, one-on-one patient care, and helping adult and geriatric patients remain safe and independent in their own homes.

Qualifications Required\n
- \n
- Graduate of an accredited Physical Therapy program
- Current New Hampshire PT license or ability to obtain prior to start
- Current BLS certification
- Previous Home Health experience
- OASIS experience
- Homecare Homebase (HCHB) experience
- Valid driver's license and reliable transportation
- Ability to independently manage a home health caseload and daily territory
- Strong clinical assessment, documentation and time-management skills

What You'll Do\n
- \n
- Complete PT evaluations and reassessments in patients' homes
- Develop individualized plans of care
- Provide gait, balance, strengthening, transfer and mobility training
- Treat orthopedic, neurological, post-surgical and medically complex patients
- Provide fall-prevention and home-safety education
- Educate patients and caregivers on home exercise programs
- Collaborate with nursing, OT, SLP, physicians and other members of the care team
- Complete timely and accurate home health documentation
