Company Description The Aldrich Group, Inc. is recruiting for a group of New Hampshire based Skilled Nursing Facilities to fill several open positions for RNs MDS Coordinators with an immediate need to bring on MDS coordinators to improve systems use at these facilities.

Role Description The MDS Coordinator is responsible for overseeing the accurate and timely completion of Minimum Data Set (MDS) assessments and ensuring regulatory compliance for residents in a long-term care or skilled nursing setting. Day-to-day responsibilities include coordinating and conducting resident assessments, collaborating with interdisciplinary team members to develop and update individualized care plans, and monitoring care plan implementation for consistency with clinical findings. The MDS Coordinator will review clinical documentation for accuracy, enter and validate MDS data in relevant systems, and prepare reports that support reimbursement, quality measures, and internal audits. The role also involves educating staff on MDS-related processes, participating in quality improvement initiatives, and supporting survey readiness and compliance activities.

Qualifications

  • Current RN in good standing with New Hampshire licensure, with experience in skilled nursing for long-term care.
  • Strong proficiency in MDS assessment and documentation, including knowledge of relevant regulation, reimbursement guidelines, and non-therapeutic ancillary.
  • Demonstrated ability in Care Planning and Care Plans development, including interdisciplinary collaboration and resident-centered planning.
  • Background in Geriatric Nursing, with skills in assessing and addressing the complex needs of older adults.
  • Excellent attention to detail, data accuracy, and organizational skills for managing multiple assessments and deadlines.
  • Effective written and verbal communication skills, including the ability to train and guide clinical staff on MDS and care planning processes.
  • Comfort with electronic health records and related clinical software; proficiency in basic office software (e.g., Excel, Word) is beneficial.
  • Ability to work independently, exercise sound clinical judgment, and maintain confidentiality and ethical standards.
  • Additional certifications in MDS coordination or gerontology are a plus.
  • Profiiency with EHR/MDS software (e.g. PointClickCFare, MatrixCare, American HealthTech).
