About the Role:
As an Evening Shift LPN, you will be responsible for providing direct patient care to our residents, including medication administration, wound care, and vital sign monitoring. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient. Your strong communication skills will enable you to effectively communicate with patients, families, and healthcare providers to ensure seamless transitions and optimal care outcomes.
Responsibilities:
